Appointment Request

New Patients: To schedule an intake assessment, individual, couples, marriage or family therapy appointment or to obtain additional information about any of these services, I prefer that you please fill out the form below or give me a call. You may also Schedule an Appointment instantly, although final approval is subject to my screening. My regular patients may schedule instantly at any time.

Please arrive on time for your appointment. If you are a new patient and have not sent in your paperwork please arrive at least 15-20 minutes prior to your appointment to complete required paperwork. If you are late, your visit will not be extended. If you have missed more than half of your appointment time (15 minutes for a follow-up medication appointment), the appointment will need to be rescheduled. This does not apply to therapy appointments. If you arrive late for your session, you will be seen for the remaining time of your appointment.
