Medication Management

It is necessary to carefully answer questions regarding past medications (name of medication, dates administered, dosage, and any reactions) in the history questionnaire. In the event that you do not remember the required information, it would be helpful to obtain such information from your doctor(s) and have it faxed directly to Dr. Malik on 310-684-2657. Pharmacies have computerized prescription records. If you can obtain one, send them also.

Dr. Malik may need to order routine lab tests and measure your vitals (blood pressure, heart rate etc.) or obtain this information from your primary care physician before starting a medication. Once a medication is started, monitoring will be required and adjustments may need to be made. The frequency of these visits will vary and are based on your response to the medication, side effects and severity of symptoms.
